Newbie With Some Cav Problems

Whats up everyone. I'm new to the site and I am having some problems with my cav. So here it goes...

97 Red 2 Door
2.2L
5 speed
Has short ram intake
test pipe
2 1/2 in Ebay brand exhaust with angled out muffler.
Eagle H beam Rods
Wiseco Pistons

Symptoms:
Car bogs down and shuts off ONLY when at operating temp and when its hot outside (say 70 degrees and up)
Its like it will drive fine for the first 30 mins or so and then all hell breaks loose.
It starts to sputter when i'm coming up to a stop sign or stop lights.
Then it will die. It immediately starts right back up but sometimes it sounds like its missing really bad.
And sometimes its starts right up and i can drive away. But a short drive later (like 3 blocks) the car will seem to lose all power
and if I try to give it more gas it completely loses power and acts like it shuts off. But when i get off the gas it will show some signs of power
but it still bogs down. And After it dies the first time, it happens a lot more often after that.
The car runs fine when its cold outside and when it also has just been started.
The car doesn't overheat. My aftermarket temp gauge shows it to only hit 190 on really hot days.

I am using an Actron CP9125 OBD2 Pocket Scan
The Codes are as follows:

P0118- -Engine Coolant Temp Circuit Low Input
P0121--TPS/ Pedal Position Sensor A CKT Range/Performance
P0140-- 02 Sensor CKT No Activity (Bank 1, Sensor 2)
P0141-- 02 Sensor Heater Circuit Malfunction (Bank 1 Sensor 2)
P0171-- System Too Lean (Bank 1)
P0342-- Camshaft Position Sensor A - Bank 1 Circuit Low Input
P0480-- Cooling Fan 1 Control Circuit
I know why the P0480 code is thrown. When i bought the car someone had
tapped a wire into the circuit for the fan and wired it to a toggle switch so it would come on on demand.
And P0118 is on because my under the hood fuse box was loose and caused the coolant temp sensor
to misread.

Replaced Parts:
All Vacuum hoses
Plugs, Wires, and Coil Packs
Fuel Filter
Crankshaft Sensor
Radiator Cap (old one had cracks in it that wasn't allowing it to hold pressure)
Flushed out Coolant when Radiator Cap was changed.

If anyone could lend a hand in figuring this out it would be greatly appreciated.
